Inschrijven patiënt
De praktijk is (beperkt) open voor nieuwe inschrijvingen.
Inschrijven is alleen mogelijk als u in Overschie of in Park 16Hoven woont en een Nederlandse basisverzekering heeft.
Graag ontvangen we de volgende documenten van u per e-mail:
– Inschrijfformulier, ingevuld en ondertekend.
– Geldig identiteitsbewijs.
– Zorgpas of zorgpolis (evt. digitaal).

Our practice is (limited) open for new registrations.
Can I register here?
– You are eligible if you live in Overschie or Park 16Hoven.
– You have a Dutch “Basisverzekering” insurance.
If you are unsure if you are eligible to register, you can e-mail us your zipcode and insurance company.
We do not make exceptions on the requirements above, please do not ask.
Note: This means that AON student insurance, Allianz Care, Cigna Global Health, Insure to Study, OOM SIN and EHIC are not accepted since we do not have direct billing contract with them.
Please return the following documents to us, and e-mail them to us:
– Registration form, completed and signed
– Valid identity document
– Insurance card or policy (digital is ok)
